Job Description
Koram Senior Housing Corporation (Koram) is a not-for-profit whose purpose is to provide subsidized housing (Section 8) for seniors and/or persons with disabilities. Koram was built in March 1987 in Albany Park and has 58 units, 44 one-bedroom and 14 studio units, including 7 wheelchair accessible units. Koram is professionally managed by HANA Center (HANA).
Position Summary:
The Service Coordinator is a critical member of the team responsible for the operations of Koram. The Service Coordinator is responsible for providing case management services to all residents living at Koram and coordinates group activities to enhance the well-being of residents.
Major Responsibilities:
Offer case management and referral services to all residents at Koram
Refer and link residents to such public benefits as SSA, SSI, Medicare, Medicaid, SNAP, LIHEAP, in-home services, adult day care, meals-on-wheels, transportation, counseling, visiting health professionals, health screening, and legal advocacy.
Organize and coordinate workshops on such areas as health, safety, leisure, physical exercise, ESL, information and technology, and other relevant topics
Work collaboratively with the site manager to organize special events like field trips, seasonal parties, music concerts, and art exhibits
Network and collaborate with other community organizations and government agencies in the field of aging
Support the residents to build informal support networks among themselves, families, and friends
Develop and publish monthly newsletters or news bulletins
Compile service statistics, maintain records of each resident served, and prepare required reports
Maintain documentation and service statistics
Follow all HUD requirements and guidelines and stays up to date on relevant changes in policies, procedures, and best practices
Perform other related duties, assigned by the HANA executive director
Qualifications:
Experience working on Korean American, Asian American, immigrant community issues, and/or social, economic, and racial justice issues preferred
Experience working with low-income seniors preferred
BA in social sciences or equivalent; MSW or MA in related disciplines preferred
3+ years work or volunteer experience in community or senior services
Bilingual proficiency in written and spoken Korean and English required
Basic computer proficiency including experience with Microsoft and databases required
Excellent time management, organization, and ability to prioritize multiple projects and deadlines
Excellent interpersonal, oral, and written communication skills
Ability to work well independently with minimal supervision and contribute collaboratively to a team
Solution-oriented mindset; demonstrated ability to take initiative, critically analyze issues, and advance innovative solutions
Flexibility to work evenings and weekends as required
Reports to: HANA Executive Director
Compensation: This is a full-time position with a salary range of $50,000-$55,000 and benefits that include paid time off, health, dental, vision, short-term disability, long-term disability, life, and 401K.
This position works in-person, Monday – Friday, 9 am – 5 pm, at Koram.
